RN Community Care
The incumbent will act as a resource to the medical center staff, contracted and networking facilities. The LOCC RN executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care within the VHA Community Care Program. This functional role includes organizing and coordinating the clinical components for the unauthorized/authorized care of Community Care services in response to individual healthcare needs along the illness and care continuum and in multiple settings. The incumbent is responsible for clinical case management activities necessary to coordinate patient care in the community and meet the Medical Center's utilization goals in a manner that successfully integrates the community care with past and future VA Care. The incumbent works collaboratively with all clinical services across the Medical Center and with the administrative staff within the LOCC to ensure coordinated, timely, cost-efficient care is provided to the Veteran. The LOCC RN frequently acts on behalf of the Medical Center and with other VISN facilities to determine availability of clinical services, ensure proper transfer of patient care, participate in collaborative case management planning, utilization review, discharge planning and other related case management activities including the review of outside medical services based on established guidelines and medical necessity. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Experience:

Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
