The incumbent will act as a resource to the medical center staff, contracted and networking facilities. The LOCC RN executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care within the VHA Community Care Program. This functional role includes organizing and coordinating the clinical components for the unauthorized/authorized care of Community Care services in response to individual healthcare needs along the illness and care continuum and in multiple settings. The incumbent is responsible for clinical case management activities necessary to coordinate patient care in the community and meet the Medical Center's utilization goals in a manner that successfully integrates the community care with past and future VA Care. The incumbent works collaboratively with all clinical services across the Medical Center and with the administrative staff within the LOCC to ensure coordinated, timely, cost-efficient care is provided to the Veteran. The LOCC RN frequently acts on behalf of the Medical Center and with other VISN facilities to determine availability of clinical services, ensure proper transfer of patient care, participate in collaborative case management planning, utilization review, discharge planning and other related case management activities including the review of outside medical services based on established guidelines and medical necessity. This position requires Basic Life Support (BLS) certification. If selected, Applicant will provide proof of current certification prior to employment. Only BLS certification which included Automated External Defibrillator (AED) certification obtained from either the American Heart Association (AHA) or the Military Treatment Training Network (MTN) will be accepted. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Experience:
2 to 3 years nursing experience in an acute care setting and/or other clinical care setting with 1 - 2 years case management/utilization review experience. Proficiency with Microsoft Office products (Outlook, Excel and Access). Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
