The Registered Nurse (RN) MDS Coordinator is ultimately responsible for adequately assessing nursing home residents' needs, and coordinating personalized, resident-driven care based on those assessments.
The Registered Nurse (RN) MDS Coordinator will report to the Director of Nursing and the primary MDS Coordinator.
The Registered Nurse (RN) MDS Coordinator position works every weekend and hours during the week to assist with the completion of resident assessments as well as floor nursing duties.
Registered Nurse (RN) MDS Coordinator Duties:
Complete care plan and resident care information sheet with new resident admissions Complete assigned assessments, sections of the MDS, and CAA summaries with new resident admissions, quarterly, upon significant changes, and annually Complete discharge and re-entry assessments Review and update care plan and resident care plan information sheet quarterly and as needed Keep and monitor RUGs worksheet for all residents on Medicare Schedule and complete all Medicare MDS Participate in Medicare meeting Monitor and complete all corrections on MDS Registered Nurse (RN) MDS Coordinator
